Consensus practice guidelines on interventions for lumbar facet joint pain developed by a multispecialty, international working group42 concluded that lumbar medial branch RFA may provide benefit to well-selected individuals, with medial branch blocks (MBB) being more predictive than intra-articular (IA) injections. More stringent selection criteria are likely to improve denervation outcomes, but at the expense of more false-negatives,42 potentially missing many patients that could benefit from RFA procedures. Intra-articular injections of corticosteroids may, however, be used as a therapeutic injection for certain patients with suspected inflammatory pain, and in those who want to avoid ablative therapies, such as young athletes.49,50
